Adult Primary Care Services with Clear Starting Options

Silva Skin Solutions offers adult primary care through two appointment options designed for different levels of need.

New patients may begin with a 60-minute Initial Adult Primary Care Evaluation for a broader review or a 30-minute Focused Adult Primary Care TeleVisit for one primary, limited, stable health concern that may be appropriate for telehealth.

Initial Adult Primary Care Evaluation — 60 Minutes

The initial evaluation is designed to begin by telehealth when clinically appropriate. It may include:

  • Review of the main reason for seeking care and current health priorities.
  • Relevant medical and health history.
  • Current medications and allergies.
  • Review of available laboratory information.
  • Preventive care needs.
  • Evaluation of common and stable adult health concerns.
  • Identification of additional testing or records that may be needed.
  • Initial treatment decisions when clinically appropriate.
  • Care coordination or referral when another level of care is needed.
  • Development of a clear initial clinical plan.

The initial evaluation does not guarantee ongoing or unlimited care.

Focused Adult Primary Care TeleVisit — 30 Minutes

The Focused Adult Primary Care TeleVisit is designed for one primary, limited, stable health concern that may be appropriate for telehealth. The clinician determines whether the concern can be evaluated virtually or whether testing, an in-person evaluation, urgent or emergency care, specialist evaluation, or another level of care is appropriate.

This is an adult primary care visit, not an urgent-care service. The clinical fee covers the evaluation performed and does not guarantee a diagnosis, prescription, refill, antibiotic, antifungal, or other specific treatment.

A Defined Initial Episode

Initial Evaluation

A 60-minute evaluation focused on understanding your priorities and creating an individualized initial plan.

If Follow-Up Is Needed

One 30-minute Follow-up TeleVisit may be included when clinically needed to complete the initial episode, either to review testing ordered by SSS or to follow up on the initial plan when clinically appropriate.

When Separate Care May Be Needed

New concerns, outside laboratory testing, additional testing cycles, continued management, or care that requires another setting may require a separate visit or another appropriate level of care.


The initial episode does not automatically establish ongoing or unlimited primary care.

Medication Review and Treatment Decisions

Medication review may include confirming the current medication list, reviewing adherence and tolerance, identifying possible duplication or inconsistency, considering monitoring needs, connecting medications with laboratory findings and current priorities, and making treatment adjustments when clinically appropriate.

A visit does not guarantee that a medication will be prescribed, refilled, continued, or changed. Silva Skin Solutions does not provide routine refills of controlled-substance prescriptions. Medication decisions are based on the clinical evaluation, and specific medications or treatments are not guaranteed without an appropriate medical indication.

Laboratory Review and Preventive Planning

When clinically relevant, care may include review of available laboratory information, identification of appropriate testing, discussion of relevant results, review of trends when sufficient information is available, and preventive planning based on the patient’s history, risks, and current needs.

When Silva Skin Solutions orders laboratory testing, SSS reviews the results it orders and communicates the result or appropriate next step. Basic review and communication of an SSS-ordered result do not by themselves create a separate visit charge. If the result or the patient’s clinical course requires a new clinical evaluation, a separate 30-minute Follow-up TeleVisit may be needed.

When laboratory testing is recommended, SSS discusses the ordered testing and the applicable patient-facing laboratory cost before the order is sent. Laboratory testing is performed by an outside laboratory; SSS does not collect blood specimens.

Common and Stable Adult Health Concerns

Silva Skin Solutions may evaluate common adult health concerns that are clinically appropriate for the practice and sufficiently stable for the planned setting. Care may include clinical assessment, initial treatment planning, medication adjustment when appropriate, monitoring recommendations, identification of warning signs, and determination of whether additional evaluation is needed.

If the evaluation determines that an in-person examination or other diagnostic testing is needed, Silva Skin Solutions will guide the patient on the appropriate next step. Some services or levels of care—such as urgent care, emergency care, or specialist evaluation—may require care outside Silva Skin Solutions.

Care Coordination and Referral

When appropriate, Silva Skin Solutions may review relevant outside information, identify when specialist evaluation may be appropriate, clarify the next clinical step, coordinate referrals, and incorporate relevant outside findings into the clinical plan.

Specialist consultations, diagnostic services, hospital care, imaging, laboratory services, and other outside services are separate from Silva Skin Solutions.

Telehealth

Both the Initial Adult Primary Care Evaluation and the Focused Adult Primary Care TeleVisit are designed to begin by telehealth when clinically appropriate. Telehealth visits are available only while the patient is physically located in Florida at the time of the visit. The patient’s stated physical location is confirmed at the beginning of the encounter. Telehealth may be used when the concern can reasonably be evaluated virtually, privacy and technology requirements can be met, and an in-person examination is not required at that stage.

Telehealth does not replace in-person care when a physical examination or other diagnostic testing is needed. If that need is identified, Silva Skin Solutions will guide the patient on the appropriate next step. Some services or levels of care—such as urgent care, emergency care, or specialist evaluation—may require care outside Silva Skin Solutions.

What the Initial Episode Does Not Include

  • Unlimited visits or multiple rounds of ongoing management.
  • Automatic longitudinal primary care.
  • New or unrelated health concerns outside the scope of the initial plan may require separate evaluation or care.
  • Open-ended clinical messaging.
  • Urgent or emergency care.
  • Chronic pain management.
  • Routine controlled-substance refill services.
  • Guaranteed prescriptions.

Additional care beyond the defined episode may require a separate visit, referral, or another appropriate level of care.

Appointments

All clinical services are provided by appointment. Public online appointment booking is available through healow for appointment types currently offered online.

New patients: may book either an Initial Adult Primary Care Evaluation — 60 minutes or a Focused Adult Primary Care TeleVisit — 30 minutes online through healow. Telehealth visits are available only while the patient is physically located in Florida at the time of the visit. Established patients: may use healow when an appropriate appointment option is available or follow scheduling instructions provided directly by the practice.
